  10. Do you find climate, specifically damp, rainy days, make your symptoms worse?

For each symptom assign the following number to your answer:
1: I do not have the symptoms
2: I have occasionally have the symptom or it is mild in nature
3: The symptom occurs often and is intense
4: I experience the symptom and it is very severe

  3. Memory loss or occasional lapses in memory
